Certificate of Added Qualification (CAQ):

Upon completion of an ASCFS-accredited craniofacial program, a fellow will be granted to opportunity to earn a Certificate of Added Qualification (CAQ) in Craniofacial Surgery*.

To qualify for a CAQ, the fellow must:

①  Complete an ASCFS-accredited craniofacial fellowship program

②  Earn a passing score (80%) on a knowledge-based exam at the end of the fellowship

③  Complete and submit a comprehensive case log meeting all required minimums.

④  Complete the fellow survey
